[Esip-infoquality] IQC work - some progress and request for action - before Feruary 2016 ends

The Google Form appears to contain entry options for each of the columns 
in the IQC Use Case Evaluation. Testing the use of the Google Form by 
entering test response values and submitting the form appeared to work, 
but I cannot verify whether the responses were received.
